is 10k cfu actually an infection?

I’ve been digging into the literature on feline urinary tract infections and I’m confused by the current clinical guidelines. My cat Olive, a 6-year-old gray shorthair, had a urine culture come back with 10,000 CFU of E. coli. The vet said it’s likely contamination or asymptomatic bacteriuria and didn’t want to start antibiotics.

I understand that high colony counts (usually >100,000) are the standard for diagnosis, but I’ve read studies suggesting that even lower counts can be significant in cats with prior urinary issues. Does anyone have experience interpreting these borderline results? I’m worried about missing an early stage infection, but I don’t want to over-treat either.

Hi Val. It is completely understandable to feel uncertain when the numbers are in that middle range. I have seen many seniors and middle-aged cats with similar results. Usually, the vet looks at the whole picture. Is Olive showing any symptoms? Straining, crying, or going outside the box?

If she is acting normal, the vet is likely right to watch and wait. You might want to look at treating vs monitoring low cfu which discusses why we don't always treat low numbers. It helped me a lot with my own cat's history.

I am also worried about this! My Bella is a 6-year-old Siamese and she has asthma which makes me super cautious about any infection. We had a similar count last year, maybe 20k? The vet said it was nothing but I was terrified it was the start of a UTI.

Does the type of bacteria matter? Is E. coli worse than other types at this low level? I feel like I should be doing something, but I don't know what. Has anyone tried adding cranberry supplements to prevent it from getting worse?

Oh honey, take a breath! My Cleo is 9 and has dental issues so I am always on edge about infections. When we got a low count like that, the vet told us it was just 'noise'.

I would just keep an eye on her litter box. If she is peeing normally and not acting weird, it is probably fine. I use unscented clay and keep it clean twice a week which seems to help her stay comfortable. Maybe stress is playing a role? Check out stress and bladder issues if you want to see how environment affects them.

From a clinical perspective, 10k CFU is generally considered below the threshold for a true infection in a catheterized or sterile specimen. In a free-catch specimen, it is even more likely to be contamination. The standard of care is to treat based on symptoms, not just the number.

If Olive is asymptomatic, antibiotics are not indicated and can disrupt the microbiome. However, it is good to monitor. If symptoms develop, such as those listed in symptoms of feline uti, then you would want to re-evaluate.

@TinaInBoise That makes sense regarding the microbiome. I appreciate the clarity.

@paulr18 I think the specific organism is less important at this count unless it’s a resistant strain, which the culture report would usually flag. I don’t know about cranberry; I haven’t found strong evidence for it in cats specifically.

I’m going to keep an eye on her water intake. She’s been a bit picky lately. Does anyone have a protocol for retesting? I don’t want to just wait months.

I would not wait months. I think a follow-up in 2-4 weeks is reasonable if you are anxious, or sooner if symptoms appear. You can look at retesting urine protocols to see what others have done.

Just remember, a clean culture doesn't mean the problem is gone forever, it just means it's not active right now. Olive is lucky to have a careful owner.

Thanks everyone. I’m going to go with the wait-and-see approach for now but will keep a close eye on her behavior. I’ll post an update in a follow-up thread in a few weeks to see if the counts change or if she develops any symptoms. For now, she’s acting normal and eating well.
